The submission must be complete and meet the underwriting guidelines and eligibility criteria.

Submissions must include the following:

• UtilitySure Supplemental Application - This application is REQUIRED with a submission and MUST be signed by the insured.
• Completed Acord applications, signed by the insured, including detailed property information to include construction, occupancy, square footage, and age.
• 4 years of currently valued carrier loss runs
• 3 years of DOT reports
• 3 years of leak surveys
• Updated driver information

If you are not yet appointed with Willis of New Hampshire, Inc., please include the following:

• Completed and signed Broker Agreement with the following:
  • Proof of E&O showing a minimum of $1,000,000 limit per occurrence.
  • Copy of agency and individual producer licenses in all states you submit business for including your resident licenses.
